Required Documents

The regular processing time is about 10 days.

Adults (age 16 and older)

All applicants must apply in person after booking an appointment with the Embassy via email at PTLConsular@state.gov or by phone on 202 4432 between 15:00 and 16:30 Monday to Thursday.

  • Present proof of U.S. citizenship
  • Present one valid proof of ID
  • Provide one passport photo (white background /2″×2″ in size / no eye glasses / no uniforms). *Starting November 1 2016, eye glasses are no longer allowed in passport photos.
  • Complete form DS-11 online, and print out.  You will sign the form before the Consular Officer.
    Please note: You must provide a Social Security number, if you have been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f).If you do not have a Social Security number, you must submit a signed statement including the phrase, “I declare under penalty of perjury under the laws of the United States of America that the following is true and correct:  I have never been issued a Social Security Number by the Social Security Administration.”
  • If your ID does not match your name that is on your proof of citizenship document, please bring  proof of your name change. Documents must be original. (e.g. Marriage Certificate, Court Order)
  • Fees:  $135 payable in US$, Mauritian Rupees or by credit card.

Children Under 16

  • All applicants (children) must appear in person accompanied by both parents.
  • Present proof of U.S. citizenship
  • Present one valid proof of ID
  • Present proof of parent-child relationship
  • Parents’ valid IDs
  • Provide one passport photo (white background /2″×2″ in size / no eye glasses / no uniforms).*Starting November 1 2016, eye glasses will no longer be allowed in passport photos.
  • Complete form DS-11 online, and print out.
    Please note: You must provide a Social Security number, if you have been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f). If you do not have a Social Security number, you must submit a signed statement including the phrase, “I declare under penalty of perjury under the laws of the United States of America that the following is true and correct:  I have never been issued a Social Security Number by the Social Security Administration.”
  • If your ID does not match your name that is on your proof of citizenship document, please bring  proof of your name change. Documents must be original. (e.g. Court Order)
  • Fees: $105 payable in US$, Mauritian Rupees or by credit card.

Children under age 16 cannot apply for a passport by themselves. Both parents/guardians must appear in person with the child and provide consent, authorizing passport issuance to the child.

If one parent/guardian is unable to appear in person, the appearing parent will need to provide either a notarized form DS-3053 Statement of Consent (PDF 50KB) with a clear photocopy of the front and back of the non-applying parent’s ID which was presented to the notary or form DS-5525 Statement of Exigent/Special Family Circumstances (PDF 42KB), depending on your circumstances.

IMPORTANT NOTE: Photocopies or scanned copies of a notarized consent form are not acceptable. Additionally, the notarized consent from must be less than three months old. Until the child reaches age 16, a new consent form will be required each time the child applies for a new passport.

If the child has only one parent/guardian, evidence of sole authority to apply for the child must be submitted with the application in the form of a:

  • Court order granting sole legal custody to the applying parent (unless child’s travel is restricted by that order)
  • A court order specifically permitting the applying parent to apply for the child’s passport
  • Judicial declaration of incompetence of the non-applying parent
  • Death certificate of the non-applying parent

 

Processing time is about 10 days

Adults (age 16 and older) application

  • You do not need to come in person to renew your passport.
  • Submit current or expired passport
  • If the most recent passport was issued within the last 15 years and when you were age 16 or over, completed form DS-82online and print out. If you are not eligible to complete form DS-82, you will need to complete and print out online form DS-11  and contact us for an appointment.
    Please note: You must provide a Social Security number, if you have been issued one, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f). If you do not have a Social Security number, you must submit a signed statement including the phrase, “I declare under penalty of perjury under the laws of the United States of America that the following is true and correct:  I have never been issued a Social Security Number by the Social Security Administration.”
  • Present one valid proof of ID

  • Fees if completing DS-82:  $110.  If completing DS-11 the fee is $135.  Payment may be done in US$, Mauritian Rupees or by credit card.

If your passport is lost or stolen, you are advised to report the loss/theft to the local Police Station and obtain a police report or a report number for presentation to the Consular Officer at time of application.

If either parent cannot be present, the appearing parent will need to provide either a notarized form DS-3053 Statement of Consent (PDF 50KB) with a photocopy of the non-applying parent’s ID presented to the notary or form DS-5525 Statement of Exigent/Special Family Circumstances (PDF 42KB), depending on your circumstances.

Can be demonstrated by the following:

  • A current valid U.S. passport
  • Certified U.S. birth certificate (should have parents’ names, a seal, registrar’s signature, and the date the certificate was filed with the registrar’s office – hospital birth certificates are not acceptable) *Birth Abstracts may not be acceptable.
  • Consular report of Birth Abroad
  • Certificate of Naturalization
  • Certificate of Citizenship

Note: all documents must be original – no photocopies.

Military ID cards and social security numbers are NOT proof of citizenship.

Can be demonstrated by the following:

  • U.S. or Foreign passport
  • U.S. or foreign Driver’s License (with photo)
  • U.S. Military ID
  • State ID (with photo)
  • Alien Resident Card (“Green Card”)
  • U.S. Government ID
  • Residence Card

Note: All documents must be valid and original.

Can be demonstrated by the following:

  • Minor’s U.S. Birth Certificate with both parents’ names
    (Hospital birth certificates are not acceptable.)
  • Minor’s Consular Report of Birth Abroad with both parents’ names
  • Minor’s Foreign Birth Certificate with both parents’ names
  • Adoption decree with adopting parents’ names
  • Court order establishing custody
  • Court order establishing guardianship
  • Note: all documents must be original – no photocopies.
